Location: 101 Progress Crescent, Kapuskasing, ON P5N 3H5

Job Type: Temporary Full-Time (12 month contract)

Compensation: Salaries range from $33.90 - $40.10 per hour.

Job Description
We are seeking a dedicated and empathetic Social Worker/Discharge Planner to join our dynamic team. The successful candidate will play a crucial role in providing an integrated model of care for palliative and end-of-life care, while promoting and enhancing the continuity and quality of care for individuals and their families through a seamless transition of care. This includes assessing patient needs, coordinating with healthcare providers and implementing timely discharge plans that enhances continuity of care and efficient use of hospital and community resources.

 

 Key Responsibilities

  • Conduct comprehensive assessments of patients ‘social, emotional, and practical needs.
  • Identifies patients who may require discharge planning on all Nursing Units, assesses their needs; plans and implements the discharge plans of patient to private homes, rehabilitation, chronic care or other facilities.
  • Collaborates with patients, families, hospital and community health care team members to provide professional service.
  • Implements discharge/transfer plans of the patient by coordinating family, hospital and community support services; makes necessary referrals. Ensures that resources are used efficiently and effectively to protect consumer interest.
  • Provides services in the areas of high-risk assessment, community referral processes, palliative care services, bereavement counseling and support, placement coordination and discharge planning.
  • Works with nursing team to develop care plans that address the psycho-social, physical, practical and spiritual needs of the client and their significant others in a holistic approach with the aim of improving the quality of end-of-life care.
  • Meets with patients and their families and empower them to be involved in informed decision-making regarding care of the patient.
  • Counsel the patient and his family in acceptance of an adaptation to changing health conditions
  • Advocate for patients’ needs and rights within the healthcare system.
  • Maintain accurate and timely documentation of patient interactions and discharge plans.

Qualifications

·         Bachelor’s or Master’s degree in Social Work from an accredited institution.

·         Licensed and in good standing with the Ontario College of Social Workers and Social   Service Workers or equivalent.

·         Previous experience in a healthcare setting, preferably in discharge planning or case   management.

·         Excellent communication, organizational, and problem-solving skills.

·         Ability to work collaboratively with healthcare professionals, patients, and families.

·         Strong knowledge of community resources and support services.

·         Compassionate and empathetic with a patient-centered approach.

·         Proficiency in both English and French is preferred.
